Institute Closing Survey

Reimbursement

We will reimburse:

  • Mileage for one vehicle per school team;
  • Up to three hotel rooms per school team for two nights, July 19th and July 20th.

Reimbursement forms are available in hard copy at the registration table, or may be completed electronically. Download the Excel Form HERE. This Excel doc has two worksheet tabs – one for electronic submission and one for print. Feel free to use the “Electronic” tab to have Excel do all of the math for you!

Please Note:

  • Along with the reimbursement form, please submit a copy of your hotel receipt showing a zero balance, and provide a google maps link or print out showing mileage.
  • Multiple forms may be submitted by each school team as the form needs to be completed by the person receiving the reimbursement check – with their name and mailing address.   If the district/school covers all expenses and reimburses their teachers, one form can be completed with district information and we will reimburse the district.
